Treatment guidelines for ASPECT scores

This scale is used in the management of acute ischemic stroke.

Regions evaluated

  • M1 – Anterior cortex of MCA
  • M2 – Lateral cortex of MCA over the insula
  • M3 – Posterior cortex of MCA
  • M4, M5, M6 – Anterior, lateral and posterior MCA territory
  • C – Caudate nucleus
  • L – Lentiform nucleus
  • IC – Internal Capsule
  • I – Insular crust

The brain is divided into 10 regions in the territory of the middle cerebral artery.

Each region is scored with 1 point if it appears normal and 0 points if it shows signs of ischemia.

  • 10 points (normal brain) 🧠
  • 0 (diffuse ischemia) to 10
  • ≤7 indicates significant ischemia 

TREATMENT GUIDE IN RELATION TO THE ASPECT SCALE

ASPECT Score: 8 - 10
Ideal candidate for reperfusion therapy
High probability of good results with thrombolysis or mechanical thrombectomy.
ASPECT Score: 6 - 7
Consider reperfusion therapy
Evaluate risk-benefit individually. Possible benefit with mechanical thrombectomy.
ASPECT Score: 6 - 7
Consider reperfusion therapy
Evaluate risk-benefit individually. Possible benefit with mechanical thrombectomy. Risk of bleeding.
