ADMINISTRATION
The decision to use phenytoin and the exact protocol will depend on your clinical judgment in each specific situation.
· Maximum infusion rate: 50 mg/minute in adults (or 1-3 mg/kg/min)
· Maintenance: Generally 4-7 mg/kg/day divided in 2-3 doses, adjusting according to serum levels and clinical response.

GENERAL CONSIDERATIONS
Monitoring:
· Continuous ECG
· Blood pressure
· Serum phenytoin levels (therapeutic target: 10-20 μg/mL).
Considerations:
· Adjust the dose in patients with hepatic or renal insufficiency.
· Monitor possible drug-drug interactions
· Watch for signs of toxicity (nystagmus, ataxia, dysarthria).
Contraindications:
· Sinus bradycardia, AV block, Stokes-Adams syndrome or porphyria.
